Select this option if you are enrolling to answer surveys and provide information for another person, including:

A person with a disease or condition, for whom you are the Parent (applies only for a child UNDER 18 OR LEGAL AGE WHERE YOU RESIDE) or Legally Authorized Representative (required for adult patients)
An adult with a disease or condition who must provide consent for themself (ie. this person is not a minor under 18 or under legal age where you reside, and you are not their Legally Authorized Representative). When adding such a patient, you must select the option "Yes, this patient will want access to their own account", even if you plan to manage their account in the future.

Once you have created your own Caregiver account and logged into the application, you will then be able to add the person with the disease or condition. When adding an adult who must provide consent for themself, you must select the option "Yes, this patient will want access to their own account", even if you plan to manage their account in the future.
